Common Translation Problems
- Summary translations: paraphrasing a discharge summary or a specialist letter loses the diagnosis-codes and drug-schedule detail the receiving clinician reads first.
- Drug-name substitution: swapping a national trade name for the closest UK trade name loses the drug the patient actually received; we preserve the INN and any national trade name and add a UK equivalent alongside.
- Untranslated reference ranges: pathology and haematology reports without their reference ranges are unusable evidence for the receiving clinician.
- Selective translation of a multi-page hospital record: leaving out consent forms, anaesthetic records or nursing notes weakens the file for both clinical continuity and litigation purposes.
- Untranslated clinician credentials or hospital seal: the letter or record's authenticity chain rests on the credentials and the seal.
